Practice Fees

Insurance: AAGPC is currently in-network with Blue Cross Blue Shield of Illinois and Cigna.

Private Pay: Services are also offered through a direct-pay system for patients who do not utilize the aforementioned insurance panels. AAGPC will provide its patients with a super-bill after each appointment to submit to their insurance provider for reimbursement.

90-minute initial evaluation appointment: $195.00

30-minute follow-up appointment: $125.00

20-minute follow-up appointment: $95.00

Payment will be rendered before each visit.
